What are my payment options at Mosaic?
Mosaic accepts most local insurance plans, including Indiana Medicaid and Medicare, as well as many commercial carriers. Ask the front desk if you would like to confirm if our providers are in-network with your insurance. If you are uninsured and choose to pay for appointments out of pocket or are covered under an insurance plan Mosaic does not accept, Mosaic is able to offer flexible payment plan options.
What is Mosaic’s Financial Assistance Program (FAP)?
Mosaic’s FAP offers uninsured patients assistance in the form of discounted medical and behavioral health services. It also offers patients the opportunity to connect with Covering Kids and Families (CKF), a non-profit organization that provides free insurance navigation.
Who is eligible for Mosaic’s Financial Assistance Program (FAP)?
Patients whose gross family income falls between 100% and 199.99% of the Federal Poverty Level (FPL) may be eligible for discounted care.
How do I apply for the Financial Assistance Program (FAP)?
  • Return the completed application to Mosaic with:
  • At least one (1) accepted income verification document: Recent Federal Tax Return IRS Form W-2 or 1099
  • Two (2) most recent pay stubs
  • Any applicable documentation to verify child support, alimony, disability benefits, etc.
  • Once processed by Mosaic, you will be referred to Covering Kids & Families (CKF), at which point you will schedule a consult to explore insurance options and eligibility.
  • Upon completed consult and receipt of application and income verification, you will receive a written determination from Mosaic.
What services are covered under the Financial Assistance Program (FAP)?
Financial assistance is available for most medical and behavioral health services offered at Mosaic. Procedures that extend beyond a regular office visit may not be included in the Financial Assistance Program (FAP). Please note that lab work and other pathology services conducted at Mosaic are not included in the Financial Assistance Program (FAP). Mosaic works with PathGroup, an outside lab who performs and bills for lab and pathology services. Questions concerning cost and payment options can be addressed by contacting PathGroup directly at (855) 627-4002.
What happens if I am not eligible for financial assistance or do not complete the application process?
If you are not eligible for the Financial Assistance Program (FAP) or do not complete the application process in full, you will be responsible for any costs associated with services received. Mosaic recognizes the difficulties medical expenses can pose and is therefore able to offer prompt pay discounts and flexible payment plans.
What happens if I am approved for financial assistance?
If you are approved for financial assistance, the cost of appointments will be reduced based on a sliding scale that follows Federal Poverty Levels (FPL). See the sliding scale on the following page. Upon approval, financial assistance is granted for a period of twelve (12) months (one year) from the date of approval.
