Telehealth Policies
We have Telehealth (video) appointments available for our current patients. Please contact your insurance provider to verify coverage.
If you have an upcoming appointment that you would like to change to Telehealth or you would like to schedule a Telehealth appointment, please send a message through the patient portal or call our office at 574-537-2680.
Reminder for Patients and Home-Based Telehealth Services:
- Ensure appointments take place in a private location
- Ideally a room with a door or a part of the home away from other members of family and as free of distractions as possible
- If patient must use a bedroom for appointment, they should be sitting at a desk or table, NOT on their bed or laying down
- Shut down all other background apps on the device to ensure the best connection
- You will need to make sure you have a reliable internet connection.
- Arrive to your appointment a few minutes early to allow for connection issues, especially if it is your first time.
- Mosaic will send a link for your appointment to your phone or email, possibly both. Click that link and you will be directed through connection tests and be connected to your provider.
